"It is each practitioner's responsibility to understand the effects each action s/he learns has on the body/mind, and to continually ask questions and deepen his/her learning. Lists or sets of general contraindication guidelines are available from a variety of sources. However, there is no clear agreement among them. The continual and on-going study of one's therapeutic actions, in relation to anatomy, physiology and pathology is a requirement for true professionalism.

The contraindications given in this course are written for the practitioners who have not previously had education in pathology or "medical massage." Practitioners who have advanced or specialized training and skills will, in many cases, be able to go beyond these recommendations.

There are two main types of contraindications discussed herein: local contraindications and systemic contraindications.

  • Local contraindications are those that relate only to a specific area of the body. Local contraindications only prohibit specific therapeutic actions to the specific location where there is an injury, pathology, or vulnerability. When an action is locally contraindicated, it means there are persuasive reasons why it is not appropriate for that specific area of the body. If other actions are NOT harmful to that area, then they are not contraindicated. An example would be a case where the client has a large bruise (ecchymosis or hematoma). We would say that applying pressure in the area of the bruise is locally contraindicated, but an action that does not apply pressure, such as energy work, could be indicated.
  • Systemic contraindications are those that relate to the client's entire body and, like the local contraindications, they relate more to specific actions or techniques than to generic terms like "massage" or "bodywork." Systemic contraindications are rare in ambulatory settings, but are more common in hospital, hospice, nursing home, and home care settings where people more often have serious systemic health problems."
